1. Application form (issued by the admissions office).
2. Copy of identity card with the original presented for verification.
3. Education document.
The following are accepted as education documents
Certificate of general secondary education.
Diploma of technical and vocational education.
Diploma of post-secondary education.
4. Medical certificate form No. 075-U and fluorography image.
5. Preventive vaccination card 063 or 065 (copy).
6. Registration certificate for male applicants.
7. Grade statement extract for applicants to “Design”.
8. UNT certificate.
9. Certificate of educational grant award.
10. Two 3x4 photos.
11. EDS key.
